As far as i know ...there is no template. I just try to use what other people do. Here is a timeless item page made by User:Papulatus and looks pretty good. Keep it similiar to it:
You can also look at most of the pages i made for armours and such. That is pretty much how you need to have them like. Hope that helps.
Here is a page i made, so keep your pages for weapons/armours similiarly made:
